TERMS AND CONDITIONS

Parent / Legal Guardian Consent - I, the Parent/Legal Guardian named above give my permission for the child named above to participate as a volunteer in the 2025 National Pharmacies Christmas Pageant.

By submitting this form, I confirm that I have read and agree to the Terms and Conditions of volunteering and will follow all SATC policies and procedures for SATC events.

I understand that the SATC will manage my personal information in line with it's Privacy Policy

I have read and understood the Volunteer Handbook.

If I feel unwell or show cold/flu symptoms, I understand it is recommended I do not attend and must notify a member of the Pageant Team as soon as possible.

I declare I am physically fit to complete the 3.5 km Adelaide Christmas Pageant route and have no known health condition that would put me at risk during the event.

Dietary & Allergy Notice – Breakfast Buffet

A breakfast buffet (including vegan, vegetarian, and gluten-free options) will be provided. SATC cannot guarantee allergy-free meals due to potential traces of allergens.
